Abstract: A 17-year-old boy presented with high-grade fever, abdominal pain, and jaundice.Laboratory investigations revealed pancytopenia, elevated lactate dehydrogenase (LDH), hypofibrinogenemia, and marked hyperferritinemia (>7000 ng/mL), a combination strongly suggestive of HLH.Bone marrow examination demonstrated above findings fulfilling the HLH-2004 diagnostic criteria.Viral polymerase chain reaction (PCR) confirmed Epstein-Barr virus (EBV) infection, the most notorious trigger for fulminant HLH in adolescents and young adults.Despite broad-spectrum antimicrobials and These microscopic findings (Figs 1 to 4) are characteristic of the hyperinflammatory cascade seen in hemophagocytic lymphohistiocytosis (HLH).To contextualize these images clinically, the following case illustrates how rapidly HLH can progress and why early marrow examination is essential.
