Abstract: Vadakkumchery Knee Osteoarthritis Physiotherapy Protocol (V-COPP): An Individualized, Multimodal Outpatient Framework Integrating Best Evidence for Knee OA CareOverviewThe Vadakkumchery Knee Osteoarthritis Physiotherapy Protocol (V-COPP) is a comprehensive, evidence-based outpatient rehabilitation protocol designed for individuals with knee osteoarthritis (OA). It was developed to address limitations of standardized group-based programs by integrating current best evidence from randomized controlled trials, meta-analyses, and major clinical guidelines.V-COPP combines manual therapy, functional exercise, kinetic chain assessment and interventions (including hip, ankle, and ITB), adjunct modalities (patellar taping, dry needling, ITB stretching), and footwear recommendations within a single, modular protocol. Patient progression is tracked using the novel, structured V-COPP Questionnaire, ensuring adaptive, patient-reported outcome monitoring.Key FeaturesIndividualized Therapeutic Approach: Allows for tailored progression based on patient-reported outcomes, pain, and functional status.Multimodal Interventions: Integrates manual therapy, exercise, adjunct therapies, kinetic chain and footwear adjustments to maximize efficacy.Dynamic Progression Tools: Utilizes a validated, patient-centered questionnaire for weekly or routine assessment to adapt care in real time.Protocol Adaptations: Offers specific guidance for complex cases, including those with severe deformity, functional loss, or perioperative needs.Evidence Synthesis: Developed following a systematic review (346 studies identified, 43 included), evaluated via PEDro and AMSTAR-2 standards.Comparison with Standard Group Programs: Directly contrasts individualized V-COPP pathways with population-level approaches like GLA:D®, highlighting flexibility and innovation.Supplementary MaterialsPRISMA 2020 Flow Table: Documents study selection and review process.V-COPP Questionnaire: Scoring tool for individualized outcome tracking.Protocol Appendices: One-page clinician reference guide for implementation.Innovation and ImpactBridges the gap between established, rigid group protocols and the evolving demands of real-world outpatient care.Provides a ready-to-implement framework that can be adopted by clinicians in diverse rehabilitation settings.Designed for immediate use, pending prospective validation, and aligns with open-access and best-practice principles.Target AudienceAllied health professionals (physiotherapists, rehabilitation specialists)Clinical researchers and educators in musculoskeletal healthOutpatient clinics seeking patient-centered, evidence-based OA managementKeywordsKnee osteoarthritis, physiotherapy protocol, V-COPP, outpatient rehabilitation, evidence-based care, manual therapy, exercise therapy, kinetic chain intervention, patient-reported outcomes, functional recovery, multimodal intervention, GLA:D®, adjunct modalities, protocol comparison, clinical implementation, outcome measurementThis project is submitted as a contemporary, evidence-driven model addressing complex patient needs in musculoskeletal rehabilitation.
