Cardio-Renal Syndrome: A Comprehensive Review of Pathophysiology, Clinical Subtypes, Diagnostic Challenges, and Therapeutic Strategies

Year: 2025
Type: article
Abstract: Background: Cardio-renal syndrome (CRS) represents a complex, bidirectional interplay between cardiac and renal dysfunction, wherein acute or chronic impairment in one organ precipitates secondary dysfunction in the other. This pathophysiological interplay significantly exacerbates morbidity and mortality, necessitating a multidisciplinary approach to diagnosis and management. Objective: This review aims to elucidate the molecular mechanisms, classification, clinical manifestations, and evolving therapeutic paradigms in CRS, while addressing gaps in current clinical practice. Methods: A systematic literature search was conducted across PubMed, EMBASE, and Cochrane databases, focusing on peer-reviewed articles, clinical trials, and consensus guidelines published between 2000 and 2023. Results: CRS is classified into five subtypes based on primary organ dysfunction and temporal progression. Emerging biomarkers (e.g., NGAL, BNP, cystatin C) and advanced imaging modalities enhance early detection. Pharmacological interventions (e.g., SGLT2 inhibitors, ARNIs, diuretics) and device-based therapies (e.g., ultrafiltration, renal denervation) show promise in mitigating disease progression. However, heterogeneity in clinical presentation and lack of standardized protocols pose persistent challenges. Conclusion: A deeper understanding of CRS pathophysiology and individualized treatment strategies are imperative to improve outcomes. Future research should focus on precision medicine approaches and large-scale randomized controlled trials to refine therapeutic algorithms.
