Abstract: Background:Gingival recessions (GR) and the lack of keratinized tissue with thin phenotype hinder proper oral hygiene. Various surgical tunneling techniques have been proposed to treat anterior mandibular GR However, the coronally advanced tunnel technique remains challenging for RT2 and RT3 recession type defects. This case series aims to present a novel technique to cover multiple adjacent recessions and modify the patient's phenotype based on the VISTA technique. Methods:A total of 6 patients presenting 30 anterior mandibular recession type defects RT2 and RT3 with thin phenotypes, were included in this study. The Split Modified VISTA technique involved the following steps: initial intrasulcular incisions, full thickness tunnel, vertical access incision, partial tunneling and a horizontal connecting incision below the tunnel. A Connective tissue graft harvested from the palate and sutured, finally the vertical incision is closed. Outcomes, including root coverage and phenotype changes, were assessed at six months postoperatively. Results:Seventeen RT2 sites achieved complete root coverage (94.4%), and ten RT3 sites achieved complete root coverage (83.3%). Overall, 90% of sites achieved complete root coverage, and 100% of patients experienced phenotype modification from thin to thick. Conclusion:The Split Modified VISTA technique represents an innovative surgical approach for achieving complete root coverage in anterior mandible. With the use of this technique, good results were achieved, reaching a 90% of complete root coverage, with higher rates of complete root coverage in the RT2 than in the RT
