Pterigion vulvo-perineal superior: una neocondición

Type: article
Abstract: Vulvo-perineal pterygium is a scar-like structure occurring at the introitus, described as a recurrent vulvar fissure or a condition in which the tissues of the vulva and perineum become thin, presenting folds or thickenings that can cause discomfort, pain, itching and even difficulty in sexual intercourse. This pathology represents a novel and understudied area, which motivated the development of this descriptive cross-sectional study to introduce the classification of a new condition: upper vulvo-perineal pterygium. The study was conducted on 130 patients who met the inclusion and exclusion criteria. After obtaining prior informed consent, clinical histories were taken, and gynecological examinations were performed. During the evaluation, the size of the membrane was measured with the Vernier caliper placed in the upper third of the vulvar area along the longitudinal axis. A new scale was developed by establishing the differences between maximum and minimum values, divided by the number of categories. The classification scale comprised three cutoff points: Type 1 (≤ 10 mm), Type 2 (10-20 mm), and Type 3 (> 20 mm). Validation revealed that 53.0% of patients were classified as Type 2, while 40.8% were categorized as Type 1. Notably, most women experiencing symptoms exhibited anterior upper vulvo-perineal pterygium Type 2. Although preliminary, these findings showcase the value of this new condition in classifying anterior upper vulvo-perineal pterygium, leading to accurate diagnosis and prompt, risk-free patient management.
