Abstract: The topic of this thesis is therapeutic alliance with patients with narcissistic personality disorders (NPD). The purpose of this paper is to investigate how such an alliance can be achieved, despite poor and diffuse prognoses and scarce guidelines. The theoretical framework is psychoanalytic and psychodynamic oriented. This paper has reviewed 6 articles, which contain 7 cases to highlight the therapeutic challenge in treatment of NPD patients. Across the seven cases the importance of projection and transferring, counter transferring, self-discloser and transparency emerge as five common factors that help promoting therapeutic alliance. The results indicate that it is difficult, but not impossible to build good therapeutic alliance with NPD-patients
