Adhésion médicamenteuse des patients atteints de trouble bipolaire : étude au Centre Hospitalier Charles Perrens, axes d'amélioration possibles à l'officine

Abstract: Bipolar disorder is a chronic psychiatric illness with manic, hypomanic, depressive and mixed episodes. Patients with this disorder have to take long-term medication with a risk of relapse at 18 months of 81.2%. In this population, 20 to 60% of patients do not take their medication as indicated on their prescriptions. This notion of taking medication as prescribed is called medication adherence. The main objectives of this thesis are to evaluate medication adherence at home of patients hospitalized for bipolar disorder by means of an interview, as well as to evaluate if the Mascard scale is usable in psychiatry. For this purpose, a study was carried out at the Charles Perrens Psychiatric Hospital on thirty patients from July to August 2021. First of all, this study shows that the Mascard scale can be used in psychiatry. Moreover, it revealed that 63.33% of the participants had difficulties in adhering to medication. We learnt that 80.00% of the patients had a pharmacy where they regularly went to when they were not hospitalized, and that the pharmacist was therefore an essential aid to good medication compliance. This study underlines the fact that patients are asking for a better pharmacist/patient relationship based on empathy. They also request the implementation of pharmaceutical interviews where their pathology and treatment will be discussed with the patient and his/her relatives. It also highlights the need for pharmacists to adopt a multi-professional approach by collaborating with psychiatrists, CMPs and, to a lesser extent, private nurses, in order to improve the management of patients with bipolar disorder.
