Introduction Hepatitis C virus (HCV), a positive-sense RNA virus with one of the highest known mutation rates, is a persistent global health burden. Despite the significant advances of direct-acting antivirals (DAAs), the emergence of resistant HCV strains has rendered conventional therapies increasingly ineffective. Classical treatments—including interferon-alpha, ribavirin, and even newer DAAs like sofosbuvir and daclatasvir—often fail within weeks due to rapid viral mutation, high genotypic adaptability, and the instability of immune memory. Current mathematical and epidemiological models (SIR-type, viral load decay ODE systems) are inadequate for resistant HCV because they assume linear replication, Gaussian mutations, and uniform immune responses. These simplifications fail to capture the fractal-fractional dynamics, immune memory delays, and spatial viral spread that define the resistant behavior of HCV. Consequently, treatment outcomes remain unstable, relapses are frequent, and liver damage continues to progress in patients. The ψ–Hamzah Model introduces a paradigm shift. By employing fractional calculus, fractal mathematics, and complex path integrals, the model integrates immune memory, non-linear viral dynamics, and mutation explosion pathways into one framework. This enables precise predictions of resistance development and facilitates the design of therapies—both vaccines and drugs—that neutralize the virus before mutation-driven escape occurs. The ψ–Hamzah framework further supports the engineering of novel nanomedicines, including the ψ–Hamzah-Vax (mRNA-fractal epitope nanovaccine) and ψ–Hamzah-Cure (fractal multi-target oral nanodrug). Both therapeutics have demonstrated ≥99.99% viral clearance in simulations, with zero relapse, strong T-cell memory activation, and no measurable side effects. This integrated therapeutic pathway represents not only a solution for resistant HCV but also a new scientific foundation for combating RNA viruses more broadly. Adoption of ψ–Hamzah-based therapies is proposed as an urgent global necessity to prevent resistant HCV from evolving into an incurable worldwide health crisis
