Abstract: Major Advisor: DejeneSeyoum (BSc, MPH /EPI) Co-Advisor:MergaChala (BSc, MSc) Abstract Background: The term premature rupture of membrane is refers to rupture of membrane before the start of labour. It varies by nation and complicates more than 4% of pregnancies globally. To reduce maternal and perinatal mortality, improving the management of premature rupture of membrane and complication prevention is crucial. However, the cause of pre mature rupture of membrane is multifactorial and not well identified in low-income countries like Ethiopia including the study area. Still there is limited information on determinant of pre mature rupture of membranes in the study area. Objective: To identify the determinants of premature rupture of membrane among pregnant women in public hospitals HoroGuduruWallaga zone, Oromia, Ethiopia Methods: Hospital based unmatched case control study was conducted from June 20, 2023 to August20, 2023 among 314 samples (cases 80 and 234 control). Cases were pregnant women with pre-mature rupture of membrane between 28-42 weeks gestation and controls were pregnant women 28-42 weeks gestational age without pre-mature rupture of membrane. Case and control proportionally allocated to each hospital. Cases were selected consecutively and Controls were selected using a systematic sampling. Data were entered into Epi-Data version 3.10 and exported to SPSS version 24 for analysis. Candidate variables were identified using binary logistic regression at P-value <0.25for multivariable analysis. Statistical significance was declared at p- value <0.05. Results: Out of 314 pregnant women, 314 (80 cases and 234 controls) were participated in the study. The result of this study showed that history of abortion [(AOR=2.4,95% CI: (1.21,4.78)] ,history of premature rupture of the membrane[(AOR=3.56,95%CI:(1.75,7.22)] ,Polyhydramnios[(AOR=3.53,95%CI:(1.67,7.43)] and under nutrition[(AOR=3.00, 95% CI: (1.62,5.57)]were determinants of premature rupture of the membrane. Conclusion: From this study history of abortion, premature ruptures of membrane, polyhydramnios and under nutrition during pregnancy were potential determinants of premature rupture of membrane. Therefore, women with a history of abortion, polyhydramnios, under nutrition and pre-mature rupture of membrane should be paid attention from health care providers to mitigate the occurrence of premature rupture of membrane.
