Type: article
Abstract: BACKGROUND: The adenomatoid odontogenic tumor (AOT) is the third most frequent odontogenic tumor originating from odontogenic epithelium. It is known to occur in three distinct forms with some being associated with impacted canine. The origin of AOT from gubernacular chord, dental lamina and dentigerous cyst is mentioned in the literature. Despite the employment of histological, histochemical, immunohistochemical and electron microscopic studies in understanding the true nature of tumor cells and mineralized components of AOT, the controversy still exists for this tumor. Recently a group of investigators have shown that cytokeratin 14 is a typical intermediate filament expressed by odontogenic epithelium and several odontogenic tumors including AOT. AIMS AND OBJECTIVES: To study A) The histopathology of adenomatoid odontogenic tumour and dentigerous cyst. B) The expression of CK14 and vimentin in Adenomatoid odontogenic tumour and dentigerous cyst. C) Elucidate the possible role of CK14 and vimentin in pathogenesis of adenomatoid odontogenic tumour and dentigerous cyst. MATERIAL AND METHODS: The retrospective study was carried out on tissue sections obtained from previously diagnosed cases of Adenomatoid Odontogenic Tumor and Dentigerous Cyst. This study group comprised of 16 cases of AOT and 15 cases of dentigerous cyst. Formalin fixed paraffin embedded tissue specimen sections were subjected to H&E, CK14 and vimentin staining. The type A and type B Cells are observed for staining characteristies and graded as negative (-), weakly positive (+), Intermediate positive (+) and strong positive (+++) for both CK14 and vimentin in AOTs. RESULTS: In study group all AOTs were predominantly found in 3 (56%) decade and seen commonly in females (69%), affecting the anterior maxilla (69%). Histologically both variants exhibited similar features of classical AOT except a few unusual findings. like melanin pigmentation and CEOT like areas. Only 50% were showing calcifications histologically. Dentigerous cyst is distributed in a wide age group, with peak in 2nd decade (40%), almost equally distributed in males and females. Histopathologically all DCs exhibited classical features. All the cases of AOT in present study expressed the CK14 positivity except one. The CK14 was predominantly expressed by type B cells than type A cells. In the present study vimentin negativity was shown in 56% (n=9) of AOTs. The CK14 expression in this study was found to be positive in all the cases of DC, except one. The vimentin expression was predominantly negative in DCs of present study and only three (3) cases being expressed positivity. CONCLUSION: The CK 14 expression profile in AOT and DC supports its odontogenic epithelial specific nature. The perforated origin of AOT and DC from REE and dental lamina is strongly supported by their intense expression of CK14. The heterogenous variable expression of vimentin in AOT suggests it to be a non specific marker but its partial role in histiogenesis of mineralized component cannot be negated. Finally the observations of this study concluded that the two distinct clinico-radiographic variants(FAOT and EAOT) exhibit similar light microscopic and immunohistochemical profile of cytokeratin 14.
