When Relief Turns to Ruin: a Case of Ketamine-induced Multi-organ Dysfunction

Year: 2025
Type: article
Abstract: Poster presented atAmerican College of Physicians (ACP) Internal Medicine Meeting 2025April 3–5, 2025 • New Orleans, LA This case report highlights the multisystem toxic effects of chronic ketamine use and the importance of early recognition and intervention. A 25-year-old male with a history of ketamine use disorder presented with flank pain, hematuria, dysuria, significant weight loss, and abnormal liver function. Imaging revealed bilateral hydronephrosis, thickened bladder, and biliary duct dilation. Laboratory findings demonstrated severe hyponatremia, acute kidney injury, and elevated liver enzymes. Multidisciplinary management included hypertonic saline, fluid restriction, percutaneous nephrostomy tubes, antibiotics, and hepatology evaluation. The patient’s stabilization and discharge underscored how timely diagnosis, supportive care, and patient education about ketamine cessation can reverse or mitigate organ dysfunction. This case emphasizes the need for clinician awareness of ketamine’s potential for renal, hepatic, urological, and systemic complications, especially with increasing recreational use.
