Year: 2025
Type: article
Abstract: Resume. Background: Ischemic heart disease (IHD) complicated by chronic heart failure (CHF) is frequently associated with psychological disorders such as anxiety and depression. Objective: To evaluate the clinical course of psychological disorders in patients with IHD and CHF during inpatient treatment and to optimize methods of pharmacological and non-pharmacological correction. Methods: A prospective study was conducted in 2022–2024 at a cardiology department including 120 patients with IHD and CHF (NYHA II–III). Patients were randomized into three groups. The Hamilton Anxiety Rating Scale (HARS) and the Beck Depression Inventory (BDI) at baseline and after 6 weeks assessed anxiety and depression. Results: Significant reduction in HARS and BDI scores was observed in all groups, but maximal improvement was in Group C. Quality of life (MLHFQ questionnaire) improved significantly in combined therapy patients compared to pharmacotherapy or psychotherapy alone. Conclusion: Comprehensive inpatient management of psychological disorders in IHD with CHF, combining pharmacological treatment and CBT, demonstrated superior efficacy and should be recommended as a standard optimization strategy.
