Zilebesiran For Hypertension Treatment: An Overview
Year: 2025
Type: article
Abstract: Hypertension remains one of the most common and serious global health problems, contributing significantly to cardiovascular and kidney diseases. Although several classes of antihypertensive drugs are available, many patients struggle with poor adherence and treatment resistance, making long-term blood pressure control difficult. Zilebesiran, a novel small interfering RNA (siRNA) therapy, offers a promising alternative by specifically targeting hepatic angiotensinogen (AGT), a key component of the renin-angiotensin-aldosterone system (RAAS). Using N-acetylgalactosamine (GalNAc) conjugation, zilebesiran achieves targeted delivery to hepatocytes, leading to sustained suppression of AGT production. Clinical studies have shown that a single subcutaneous injection can reduce both systolic and diastolic blood pressure for up to six months, with an encouraging safety profile. This infrequent dosing schedule may overcome adherence issues associated with daily oral medications, positioning zilebesiran as a potential breakthrough in the management of hypertension.
