The Role of Ivabradine in Heart Failure with Reduced Ejection Fraction: A Comprehensive Review

Year: 2025
Type: article
Abstract: Background: Heart failure with reduced ejection fraction (HFrEF) represents a complex clinical syndrome characterized by neurohormonal activation and autonomic dysregulation, wherein an elevated resting heart rate is a well-established independent prognosticator of adverse cardiovascular outcomes. This review delineates the pathophysiology of sinus node dysfunction in HFrEF and the specific electropharmacological profile of ivabradine, a novel agent that selectively inhibits the I*f* current in the sinoatrial node, thereby inducing pure heart rate reduction without negative inotropic or lusitropic effects. The discourse progresses from the foundational SHIFT trial, which demonstrated a significant reduction in the composite endpoint of cardiovascular death and hospitalization for worsening heart failure in patients with a sinus rhythm heart rate ≥70 beats per minute, to subsequent real-world evidence and sub-analyses that have refined its application. We synthesize the current evidence positioning ivabradine within the contemporary, rapidly evolving guideline-directed medical therapy (GDMT) paradigm for HFrEF, highlighting its role in patients who remain tachycardic despite maximally tolerated beta-blockade. The safety profile and specific patient phenotypes most likely to derive benefit are also examined, providing a holistic overview of its place in modern cardiovascular therapeutics.
