Abstract: Background:This study aimed to investigate the prevalence of abnormal prenatal ultrasound findings and evaluate associated maternal and fetal outcomes at Kermanshah University of Medical Sciences and Imam Reza Hospital during 2022–2023. The primary goal was to assist in more accurate clinical decision-making and to reduce maternal anxiety when facing abnormal ultrasound results. Methods:This descriptive cross-sectional study was conducted on 97 pregnant women with abnormal ultrasound findings. Sampling was performed using the census method. Data were collected through a standardized checklist and face-to-face interviews. Ultrasound findings were confirmed by a perinatologist. Data analysis was performed using SPSS version 26 with Chi-square and independent t-tests. Results:The mean age of the mothers was 31.78 years. The most common abnormalities were echogenic intracardiac focus (29.9%) and choroid plexus cysts (25%), mostly observed in the left ventricle. Fetal outcomes included intrauterine growth restriction (23.7%) and NICU admission (18.6%), while 40.2% of cases showed no specific fetal outcomes. Regarding maternal outcomes, the most common were hypertension (22.7%) and preterm labor (13.4%), with 43.3% reporting no specific complications. A significant relationship was found between the type of fetal anomaly and outcomes such as IUGR and intrauterine death, but not with maternal outcomes. Conclusion:Prenatal ultrasound plays an important role in the early detection of soft markers. While some findings were associated with adverse outcomes, many were benign with no negative consequences. Therefore, cautious interpretation and avoidance of unnecessary interventions—especially in low-risk cases—are recommended.
