Abstract: Dissemination of the Pal-Cycles project at the 7th International Seminar on Palliative Care, held on 26 September 2025 at FMUP (Faculty of Medicine, University of Porto). The Faculty of Health Sciences and Nursing represented by researcher Sofia Nunes, who presented the Pal-Cycles project in Pitch: Innovation in Palliative Care. The brief and objective presentation highlighted the proposal to develop and evaluate an integrated, person-centered model for palliative care, with a focus on improving the continuity and quality of the transition of cancer patients from hospital to community, promoting more humane and sustainable care.
