Clinical Pharmacology of Psychotropic Drugs: Mechanisms, Metabolism, and Therapeutic Application
Year: 2025
Type: article
Abstract: Psychotropic drugs represent a cornerstone of modern neuropsychiatric therapy, encompassing a diverse spectrum of agents that influence mood, cognition, perception, and behavior through modulation of central neurotransmitter systems. This review provides a comprehensive clinical and pharmacological analysis of six major psychotropic classes—psychostimulants, neuroleptics, tranquilizers, antidepressants, hypnotics, and antiepileptic drugs—with emphasis on their mechanisms of action, pharmacokinetics, pharmacodynamics, and therapeutic indications. The discussion explores receptor-binding profiles (dopaminergic, serotonergic, adrenergic, GABAergic, and glutamatergic systems), CYP-mediated metabolic pathways, half-lives, elimination routes, and the influence of genetic and age-related variability on drug selection and response. Particular attention is given to clinical principles guiding dosage adjustment, polypharmacy management, and monitoring for neuroleptic malignant syndrome (NMS), metabolic complications, and neurotoxic reactions. Additionally, the review highlights methods for evaluating efficacy and safety, including validated clinical scales, laboratory assessments, and the emerging role of inflammatory biomarkers such as the neutrophil-to-lymphocyte ratio (NLR). Through integration of pharmacological mechanisms with clinical decision-making, this article aims to provide clinicians and trainees with a structured framework for rational psychotropic drug therapy—balancing efficacy, safety, and patient individuality.
