Abstract: The main problem of out-patient haematological is infectious complication for haematological diseases and specific drugs used, both responsible for profound immunosuppression. In addition, the prolonged survival, makes patients chronic with exposure to pathogens for a long time. We report the more frequent causes of infections in a cohort of consecutive patients treated as out-patients in different haematologies of Rome and hospitalized in our institution for infective complication. Patients and Methods: Between February 2023 to December 2024, 248 haematological pts were hospitalized in our institution with medical complications. Ninety of them (36.3%) were admitted for infections. Characteristics of patients with infections are reported in table. The most frequent site of infection was lung or superior respiratory tract (54.4%). Sepsis with positive blood cultures were observed in 10 out of 90 pts (11.11%) and FUO in only 4 (5.56%). In all patients, antibiotic treatment lasted almost 5 days in lung infections or 7 days in case of sepsis, and/or almost 2 days after the disappearance of fever. In case of resistance of antibiotics, treatment was changed to alternative drugs and/or following hemo-cultures. Of all 90 pts 4 (1 Multiple Myeloma, 1 Chronic Lymphocytic Leukaemia, 1 Myelodysplastic Syndrome, 1 Acute Leukaemia) died, 6 were sent to the emergency room because complications that cannot be dealt with in our facility and lost at follow-up, 2 pts were sent to palliative care setting. The remaining 68 pts, after resolution of the acute complication, were referred, sometime after our ambulatory check after the discharge, to their reference haematologists for continuing specific treatment or follow-up. Discussion and conclusions: Proper assistance and cure of haematological patients with disease or treatment complication is become a true emergency for more and more frail pts are treated in ambulatory or day hospital setting and for the ageing of population. A local network for the care and the admission of these patients in ward multiskilfull to face different occurring complication is crucial in big cities in order to accelerate the reception and proper treatment of these conditions that can be fatal if caught late.
