P166 | Real-world Clinical Practice in Italian Patients With Multiple Myeloma: Preliminary Analysis of the My Myeloma Multicenter Registry

Year: 2025
Type: article
Abstract: Background. The Italian, non-interventional, multicenter registry MY MYELOMA aims to monitor real-world clinical practice for multiple myeloma (MM) patients (pts). We report the preliminary analysis of the first 1049 registered pts. Methods. Pts diagnosed with symptomatic MM since 1/1/2019 were eligible. Pts who signed informed consent within 90 days of diagnosis were included in a prospective cohort, while the remaining pts, including deceased pts, were included in a retrospective cohort. Baseline data included diagnostic/prognostic information (i.e., cytogenetic data). For each therapy line, we recorded treatment type, response and adverse and survival outcomes. For pts participating in interventional studies, only survival data and treatment type were collected. Data on perceived health and financial impact are being collected every 6 months. Amgen, BMS, GSK, Johnson & Johnson, Sanofi and Takeda provided the funding to conduct the registry. Results. Since March 2021, 1049 pts from 24 Italian centers were included: 495 in the retrospective and 554 in the prospective cohort, respectively (data cut-off Jan 3, 2025). The median age was 67 years, with 69% and 59% of pts aged ≤70 years, respectively. The IMWG frailty score was used in only 5% and 9% of pts. Overall, FISH analysis was performed in 62% of pts, mostly in younger pts (67% aged ≤70 years vs 52% >70). ASCT was performed in 43% (30% single ASCT, 13% double ASCT), with a median age of 60 years (IQR 54–65). As 95% of retrospective pts were diagnosed between 2019 and 2021 and 95% of prospective pts from 2022 and 2024, we analyzed how treatment practices evolved over time. First-line regimens included mAbs (mainly daratumumab [Dara]) in 23% of retrospective and 87% of prospective pts (p<0.001). Among transplant-eligible pts, VTd was the most used regimen in the retrospective cohort (38%), while DaraVTd the most used in the prospective cohort (42%). Among transplant-ineligible pts, Rd and VMP were used at similar rates in the retrospective cohort (10% vs 9%; p=0.9), while DaraRd was more commonly used than DaraVMP in the prospective cohort (28% vs 8%; p<0.001). Conclusion. In this real-world population, FISH analysis was performed in ~60% of pts, mostly in younger pts, while the IMWG frailty score was not routinely assessed. Since 2021, in line with the Italian approvals, anti-CD38 mAbs have been increasingly used in first-line regimens (DaraVTd and DaraRd). Patient inclusion in the registry is ongoing.
