Purpose: The increasing occurrence of precocious puberty necessitates accurate diagnosis of central precocious puberty (CPP). While the gonadotropin-releasing hormone (GnRH) stimulation test remains the gold standard for diagnosing CPP, the restricted availability of gonadorelin has prompted the use of triptorelin as an alternative. This study aimed to evaluate the comparability of the triptorelin test to the gonadorelin test in diagnosing CPP in boys exhibiting early signs of puberty. Methods: We conducted a retrospective analysis of 46 boys under the age of 9, with advanced bone age and a testicular volume ≥4 mL. Participants were divided into two groups: 23 boys underwent the gonadorelin stimulation test, and 23 underwent the triptorelin stimulation test. We measured luteinizing hormone (LH) levels at 30, 45, 60, and 90 min post-injection to compare the results. Results: There were no significant differences in clinical features, including age, height, weight, body mass index, testicular volume, and basal levels of LH, follicle stimulating hormone, and testosterone between the groups. However, peak LH levels were notably higher in the triptorelin group (20.0 mIU/mL) compared to the gonadorelin group (9.7 mIU/mL). In the gonadorelin group, 69.6% of participants reached peak LH levels at 45 minutes. Meanwhile, 87% of the triptorelin group reached their peak at 90 minutes, with LH levels still rising at this time point. Conclusions: The triptorelin stimulation test shows limitations as a reliable alternative to the traditional gonadorelin stimulation test in boys. Results from the triptorelin test should be interpreted with caution due to these differences. Presentation: Sunday, July 13, 2025
