Background: Non-insulin dependent diabetes mellitus (E11.9) remains a major public health burden, contributing to significant morbidity and mortality. Understanding the geographic, gender, and racial variations in diabetes-related mortality is essential for guiding targeted interventions and healthcare policies. Methods: A retrospective analysis of diabetes-related deaths from 1999 to 2020 was conducted using CDC WONDER data. Crude and age-adjusted mortality rates (AAMR) per 100,000 population were analyzed, stratified by state, gender, and race. Geographic and demographic patterns were examined to assess disparities in mortality rates. Results: A total of 294,827 diabetes-related deaths were identified. Geographic analysis revealed that West Virginia had the highest AAMR (7.7 per 100,000), followed by Oklahoma (5.9 per 100,000) and Ohio (5.8 per 100,000). Racial disparities were also evident, with American Indian or Alaska Native populations exhibiting an AAMR of 7.0 per 100,000, the highest among all racial groups. Gender analysis demonstrated that females experienced a higher burden of diabetes-related mortality (23,000 deaths) compared to males (18,000 deaths), highlighting a need for gender-specific prevention strategies. The findings emphasize persistent disparities in diabetes-related mortality, underscoring the necessity for improved healthcare access and targeted prevention strategies for high-risk populations. Conclusion: Significant geographic, gender, and racial disparities exist in diabetes-related mortality, with West Virginia, American Indian/Alaska Native populations, and females experiencing the highest burden. These findings underscore the urgent need for equitable healthcare initiatives, early screening programs, and culturally tailored interventions to mitigate the impact of diabetes-related mortality.
