Abstract: Disclosure: B. Venkatesan: None. Background: The management of thyroid cancer involves surgery followed as needed by other treatments such as radioactive iodine therapy, and targeted chemotherapy or immunotherapy. Current guidelines from the British Thyroid Association (2014) and American Thyroid Association (2015) recommend thyroid lobectomy alone for low-risk differentiated thyroid cancers measuring 1 to 4 cm. Multiple studies worldwide have conflicting results: some support thyroid lobectomy, while others indicate these patients have an increased risk of recurrence in the contralateral lobe. Objective: To analyze the indications and recurrence rate for total thyroidectomy (TT) versus thyroid lobectomy (TL) in patients with low-risk well-differentiated thyroid cancer based on current guidelines and to analyze patterns of recurrence among different ethnic groups. Methods: We conducted a single-center, retrospective chart review of patients 21 years or older who underwent surgery for well-differentiated thyroid cancer at South Brooklyn Health between January 1, 2003, and October 30, 2023. Results: The analysis included 48 patients who underwent thyroidectomy. Of the 48 patients, 26 (54%) had low-risk thyroid cancer and in that group 14 (56%) underwent TL (5 before 2015 and 9 after 2015) and 12 (46%) underwent TT (3 before 2015 and 9 after 2015). Of the 18 low-risk patients who underwent TL (9) and TT (9) after 2015, 7/9 (78%) and 8/9 (89%) had nodules ≤4 cm respectively. Overall, 1 of 14 patients (7%) who had TL had recurrence compared to 4 of 12 patients (33%) who had TT. Our study population consisted of Hispanic (48%, n=23), Caucasian (27%, n=13), South Asian (17%, n=8), and Other (8%, n=4) patients. Notably, 4 of the 5 patients who experienced disease recurrence were Hispanic, with the remaining patient from the Other group. Conclusions: Our observations indicate that about almost half the patients with low-risk, well-differentiated thyroid cancer underwent TL after 2015 and observed to have low recurrence rate. The Hispanic population demonstrated higher rates of locally advanced disease and recurrence, suggesting a potentially more aggressive disease nature and/or limited awareness of their medical condition.
