"Architecture Failures in AI Platforms: A FRA-Based Audit

Year: 2025
Type: article
Abstract: AI platforms are facing unprecedented complexity, and “old-school” approaches just don’t work anymore. Why do outages and unpredictable failures keep happening in large AI platforms? In this report, we break down technical failures across OpenAI’s ChatGPT, Sora, and related services — but not just by listing symptoms. We applied a systems analysis approach that focuses on: Not only the surface bugs, but deep architectural patterns, The real impact of human decision-making, not just “algorithms”, How “interface chaos” between components causes most of the critical breakdowns, And why old-school “add more servers” thinking is no longer enough for AI. Our perspective is unconventional: we used a fractal (FRA-inspired) lens to map how complexity, overload, and feedback gaps trigger the “avalanche effect” — and why this is a warning for all scalable AI products. The report is technical, but written for both engineers and product owners — with detailed checklists, real-world failure patterns, and recommendations for building truly resilient AI systems. What’s inside: • How failures propagate across interconnected modules • Where monitoring and security protocols break down • The hidden role of human (not just machine) choices in engineering crises • Concrete steps to make large AI platforms robust — before the next wave of outages Why this report is different: – Combines technical rigor with systemic, “pattern-level” thinking – Shows how feedback loops (not just bugs) drive instability – Written independently, no marketing — only facts, diagnostics, and engineering advice – This is a guide for anyone who wants to build, maintain, or audit modern AI platforms in the era of explosive complexity.
