Response to “A clinical ticking clock: When the SECS interval becomes a surgical risk”

Abstract: Dear Editor, We are very happy to note the thoroughness with which our article has been examined. We agree that some of our results were unexpected and have tried to explain them by studying other such studies from the Subcontinent. The term “good vision” is restricted to only the visual acuity of the operated eye and not to other aspects of vision. Furthermore, this is as opined by the patients, and may not be translated to a specific value of recorded visual acuity. When we interviewed the patients regarding their 1st eye surgery and the time gap to their presentation for the 2nd eye, 171 (45.5%) patients said they had not perceived the need for 2nd eye surgery earlier, as the vision in the operated eye was adequate for their requirements. We accept that these patients would not be aware of the lack of binocular vision. Some reasons we postulate for this are elderly dependent patients who may be satisfied with the improvement in only visual acuity and with family support or nonrequirement for activities like driving. Illiteracy and accompanying lower demand for binocular vision in daily activities may also be a contributing factor. However, we would like to draw attention to Figure 1 in the original manuscript, which shows the most frequently observed time gap between the first and second eye surgery. It can be observed that 75% of the patients were operated for the second eye within 2 years of their first eye. We are in agreement that not only should the patients be sensitized to the need for an early 2nd eye surgery for stereopsis but also that “human hurdles” should be addressed. This should probably be addressed as soon as possible during a patient interaction after the 1st eye surgery. Thanking You. Data availability statement Not applicable.
