Year: 2025
Type: article
Study design: Case Report
Abstract: room air, random blood sugar (RBS) 248 mg%, temperature (T) 98°F, and jugular venous pressure (JVP) elevated. On auscultation, crepitations present in bilateral basal lung fields. No peripheral edema, pallor, or cyanosis. Investigations• Arterial blood gas (ABG): Chronic respiratory alkalosis.Troponin T (Trop T) positive.B-type natriuretic peptide (BNP) 2430 pg/ mL.Liver function test (LFT) deranged [serum glutamate pyruvate transaminase (SGPT)/serum glutamate oxaloacetate transaminase (SGOT) >500 IU/L].Renal function test showed raised creatinine, requiring hemodialysis.• Echo: Global hypokinesia, severely reduced EF (10%), dilated LA, no clot or vegetation. ManagementPatient was treated with direct current (DC) cardioversion (150-200 J), intravenous (IV) amiodarone infusion, inotropes (dobutamine), hemodialysis, and renal-adjusted antibiotics.
