Abstract: A 63-year-old male patient underwent cataract surgery 1.5 years back and came with pseudoexfoliation material arranged uniformly in a spoke-like pattern on posterior chamber intraocular lens (IOL). As shown in Figure 1 presence of pseudoexfoliation material in pseudophakia is a rare finding[1] and commonly seen as ‘Target sign’ in phakic patients. Pseudoexfoliation in pseudophakia is infrequently noted as the distance between the posterior iris epithelium, and an IOL is too large to allow deposition of pseudoexfoliation material.[2] Pseudoexfoliation is a common cause of secondary open-angle glaucoma, thus showing the need for vigilant examination for pseudoexfoliation in pseudophakic patients, since the deposits on PCIOL may be the only sign to diagnose secondary open-angle glaucoma.Figure 1: (a) Slit lamp examination showing pseudoexfoliation on PC intraocular lens (arrow) in direct focal illumination. In the form the patient(s) has/have given his/her/their consent for his/her/their images and other clinical information to be reported in the journal. The patients understand that their names and initials will not be published and due efforts will be made to conceal their identity, but anonymity cannot be guaranteed.
