Ileal conduit complications. Management strategies
Year: 2025
Type: article
Abstract: The ileal conduit is the most commonly performed urinary diversion following radical cystectomy, yet it is associated with a high complication rate, reaching up to 77 %. Complications are typically categorized by anatomical location, including the uretero-ileal anastomosis, the stoma, and the ileal segment. This article showcases the robotic management of ileal conduit complications across diverse clinical scenarios, integrating insights from a literature review with detailed video presentations of complex clinical cases. The cases included uretero-ileal anastomosis strictures, ileal segment complications, and stoma-related complications. Surgical steps with technical tips are described for each case. Robotic-assisted uretero-ileal anastomosis reconstruction was successfully performed in two cases. A case of parastomal hernia repair included mesh placement; however, postoperative bowel obstruction required laparoscopic exploration, bowel resection, and stoma relocation. Robotic-assisted surgery offers precise and effective management for ileal conduit complications. However, these complex procedures should be performed in centers with expertise to optimize patient outcomes.
