Enhancing Ophthalmic Treatment through Ocular Microbiology: Insights from a Cornea Clinician
Year: 2025
Type: article
Abstract: Purpose: To describe the essential role of microbiology laboratories in the diagnosis and management of infectious eye diseases and to emphasize the value of coordinated clinical–laboratory collaboration in improving patient outcomes. Methods: This narrative review integrates clinical experience from a cornea specialist with published evidence on microbiological diagnostic techniques, antimicrobial susceptibility testing, and molecular methods. Key aspects examined include corneal scraping, culture, PCR-based diagnostics, and the role of microbiological monitoring in treatment planning. Results: Microbiology laboratories enable rapid and accurate identification of bacterial, fungal, viral, and parasitic pathogens through microscopy, culture, sensitivity testing, and PCR. Laboratory guidance improves antimicrobial selection, supports monitoring of treatment response, and aids in detecting resistant or recurrent infections. Advanced diagnostics such as corneal biopsy and molecular assays enhance detection in refractory keratitis. However, disparities in laboratory availability, particularly in resource-limited settings, contribute to delayed diagnosis and poorer outcomes. Conclusion: Effective integration of microbiological findings with clinical judgment is vital for targeted therapy and improved visual prognosis in infectious keratitis. Strengthening diagnostic infrastructure and fostering clinician–microbiologist collaboration are essential to ensure timely, equitable, and evidence-based ophthalmic care.
