Assessment of lateral intercostal artery perforator flap in partial breast reconstruction after breast conserving surgery (BCS) for cancer breast patients.

Type: article
Abstract: Background: To optimise cancer treatment and aesthetic outcomes, oncoplastic breast surgery (OPBS) blends plastic surgical techniques with oncological resection. In breast-conserving surgery (BCS), the lateral intercostal artery perforator (LICAP) flap is a helpful option for volume restoration, particularly when extensive tissue excision is required. Methods: A prospective study was conducted on 20 female patients undergoing BCS with significant volume defects. Patient demographics, clinical characteristics, surgical details, and postoperative complications were recorded. The LICAP flap was designed based on preoperative Doppler assessment and intraoperative vascular considerations. Results: The mean patient age was 49.4 years, and the mean BMI was 26.8kg/m². Tumors were predominantly located in the upper outer quadrant (55%) and staged as I (55%) or II (45%). The LICAP flap was successfully utilized in all cases, with minimal complications (5% flap necrosis, 20% seroma). Cosmetic evaluation showed 60% excellent, 35% good, and 5% fair results. Patient satisfaction was high (95%).Conclusion: The LICAP flap is a reliable choice for volume replacement in OPBS, providing good cosmetic results with minimal side effects. It corrects volume deficiencies while maintaining vascular integrity, making it a useful approach for reconstructive breast surgery.
