Abstract: Sexual and gender minorities (SGMs) have continuously been found to experience poorer mental health outcomes compared to the general population, despite changes in attitudes and legislation concerning these minorities having taken place throughout the 21st century. The present study was one of the first to assess group level changes in mental health and well-being among SGMs compared to their heterosexual, cisgender counterparts. Four waves of a large, Finnish population-based data collection spanning 16 years was utilised to compare heterosexual and sexual minority adults, as well as cisgender and gender minority adults on symptoms of depression and anxiety, alcohol use, and sexual distress. Results showed that SGMs reported more symptoms of both depression and anxiety relative to their cisgender heterosexual counterparts. Symptoms of both depression and anxiety showed increases in both the minority groups and the general population. While SGMs reported more alcohol consumption at baseline compared to the general population, consumption decreased over time in all groups except the cisgender comparison group. Sexual distress did not change significantly over time in sexual minority or heterosexual adults, although the former reported higher baseline levels. In the comparison of gender minority and cisgender adults, the former group reported more sexual distress at the first time point, followed by a convergence of scores. The present study calls for more research focus on different minority subgroups, and continued investigation of both the reasons behind and solutions for continued mental health disparities between SGMs and the general population.
