Abstract: Introduction: PPA is a rare, heterogeneous neurological syndrome manifested by language impairment alone. Language disorders have also been described in certain neuro-evolutionary pathologies such as AD and FTD. At present, there are few French studies comparing language performance in PPA, AD and FTD using a single battery. Objectives: The aim of this study was to compare the language profiles of patients with PPA, AD and FTD using the French GréMots battery. Method: We retrospectively studied 211 speech and language therapy assessments performed with GréMots. The population recruited was divided into 5 groups: 35 svPPA, 42 nfvPPA, 50 lvPPA, 69 AD and 15 FTD. The raw scores obtained on the various tests were then studied, in particular using the dual language pathway model. Three analyses were performed: intra-PPA comparison, inter-PPA comparison and comparison between PPA and other pathological groups without language presentation. The results were presented in the form of boxplots. Results: The GréMots battery includes tests that are sensitive to inter- and intra-group comparisons. AD and FTD performed better than PPA. The svPPA stood out for its lower scores in comprehension and lexical-semantic tests. The nfvPPA performed less well than the other groups in certain tests assessing the motor, grammatical and phonological aspects of language. lvPPA had no single test isolating it from the other subgroups, but working memory was impaired. Conclusion: The language components underpinned by the dorsal language pathway appear to be deficient in nfvPPA and lvPPA, while the components underpinned by the ventral language pathway appear to be deficient in svPPA. The GréMots is an interesting tool for assessing language differences and therefore making a diagnosis of PPA.
