Abstract: Verrucous psoriasis is a rare and atypical variant of psoriasis that exhibits overlapping clinical and histopathologic features of psoriasis and verruca vulgaris.A 19-year-old man with no significant medical history presented with hyperkeratotic verrucous plaques predominantly on the extensor surfaces of the arms and legs.Histopathologic examination revealed hyperkeratosis, loss of the granular layer, acanthosis, elongation of rete ridges, Munro microabscesses, papillomatosis, and epithelial buttressing consistent with verrucous psoriasis. The patient was treated with oral acitretin, topical corticosteroids and vitamin D analogs, followed by cyclosporine, resulting in gradual improvement. This case highlights the importance of distinguishing verrucous psoriasis through clinicopathologic correlation.Continued reporting of similar cases will aid in understanding the disease mechanism and establishing optimal treatment strategies.
