Surgical Management of Uncommon Spinal Osteochondromas in Two Distinct Cases

Abstract: Spinal osteochondromas are often challenging to diagnose and manage due to their location near delicate neurological structures. We present two cases of spinal osteochondromas managed surgically. Case 1: A 63-year-old male presented with left upper limb radiating pain and gait difficulty. Imaging revealed a bony outgrowth at C4–C5 facet joint, causing cord compression and foraminal stenosis. He underwent a decompressive laminectomy from C3 to C6 and fixation from C3 to C7. Case 2: A 40-year-old male presented with severe left lower limb pain. Imaging showed a bony outgrowth from the right 9 th rib head, extending into the spinal canal and causing severe cord compression. He underwent D9–D10 laminectomy, excision of the mass, and D8–D10 pedicle screw fixation. Both patients showed significant improvement postoperatively, with resolution of pain and gait difficulty at the 24-month follow-up. Early diagnosis of the lesion and appropriate surgery with meticulous excision of the tumor, along with complete resection of the cartilaginous cap, in symptomatic patients, provides an excellent outcome and is important in preventing recurrence.
