Relevance of weight-bearing long leg radiographs in the management of patients with arthritic knees

Year: 2025
Type: article
Abstract: Full-length weight-bearing long leg radiographs (LLRs) may be obtained through conventional or digital radiography, using a single long cassette, by stitching multiple images, or with the help of a specialized imaging system that uses fan-beam technology. In the management of early knee arthritis, they aid in assessing lower limb alignment, choosing appropriate modalities of conservative or surgical treatment, and monitoring disease progression over the course of time. Before total knee arthroplasty (TKA), they help evaluate the native lower limb anatomy and deformity characteristics, plan appropriate alignment strategies and implants, and anticipate required soft-tissue releases and variations in intraoperative reference points for intra- or extramedullary jig placement, thereby optimizing surgical workflows in a cost-effective manner. Once physiologic weight-bearing is achieved after TKA, they help assess surgical accuracy in terms of achieved component positioning and alignment, calculate postoperative outcome scores, and identify early signs of deformity progression and impending failure. Weight-bearing long leg radiographs (LLRs) remain relevant in the management of patients with knee arthritis and serve a synergistic role even in the age of technology-driven surgery due to the vast volume of valuable information they provide and the absence of a more accurate cost-effective modality without undue radiation exposure.
