199 Improving Timeliness and Availability of Bariatric MDT Outcomes on Electronic Patient Records: A Quality Improvement Initiative
Year: 2025
Type: article
Abstract: Aim To improve the documentation and timeliness of bariatric multidisciplinary team (MDT) outcomes on electronic patient records (EPR), ensuring clinicians have prompt access to care plans and reducing delays in patient management. Method A retrospective audit was conducted of all patients discussed at bariatric MDT meetings between 2023 and 2024. Data were collected on the presence of an MDT outcome on the EPR and the time delay between the meeting date and EPR entry. In January 2025, a quality improvement intervention was introduced: live documentation of MDT outcomes during the meeting. The audit cycle was repeated for patients discussed between January and May 2025. Results In the initial audit cycle (n=1499), 227 patients (15.3%) had no MDT outcome documented on the EPR. Among the 1272 patients with a report, only 8.5% (n=108) had same-day entries, with an average delay of 36 days (range 0–550). post-intervention (n=294), only 11 patients (3.7%) lacked an EPR report. Of the remaining 279 patients, 43.4% (n=121) had same-day entries, and the average delay decreased to 3.8 days (range 0–121). Conclusions Live reporting of bariatric MDT outcomes significantly improved documentation rates and reduced reporting delays on EPR. This intervention enhances timely access to patient management plans, promoting more efficient and coordinated care.
