Abstract: Background: Hypertension is a common comorbidity in patients with chronic kidney disease (CKD), significantly contributing to disease progression and higher cardiovascular risk. Aims and Objectives: This study aimed to compare the antihypertensive effectiveness, kidney-protective effects, and safety profiles of telmisartan and cilnidipine in patients with hypertensive CKD. Materials and Methods: A prospective observational study was conducted over a 6-month period at Sree Shakthi Nursing Home in Yemmiganur. A total of 100 CKD patients with hypertension were enrolled and divided into two groups: Group A received cilnidipine 10 mg once daily, and Group B received telmisartan 40 mg once daily. Outcomes measured included changes in blood pressure, renal function parameters (serum creatinine, urea, uric acid, and proteinuria), and the incidence of adverse effects. Results: Both groups showed significant reductions in blood pressure and improvements in renal function markers. However, the cilnidipine group experienced a more notable decrease in systolic blood pressure, serum creatinine levels, and proteinuria compared to the telmisartan group. Additionally, fewer adverse effects, such as dizziness and fatigue, were reported in the cilnidipine group. Conclusion: Cilnidipine proved to be more effective than telmisartan in lowering systolic blood pressure and enhancing renal outcomes in hypertensive CKD patients, with a better safety profile. These findings suggest that cilnidipine may be a preferred treatment option for this population, although larger randomized studies are needed to confirm these results.
