Leaving better managed than when they arrived: A window of opportunity for people with diabetes admitted to hospital
Year: 2025
Type: article
Abstract: Hospital inpatient services specialise in treating acute conditions, exacerbations of chronic conditions and peri-operative care. In contrast, chronic conditions are best managed in ambulatory care contexts, either entirely in primary care or including specialist input. Hospital ad missions, though primarily focused on the acute condition, present an opportune occasion where the interaction between patients with chronic conditions and specialists allows for management optimisation. This opportunity applies especially to chronic conditions such as diabetes, which are frequently exacerbated by the acute illness occasioning hospitalisation.
