Abstract: Background: The development of acute pancreatitis (AP) is common. There is a reported range of 13–45 cases/100,000 persons per year. Because of the significant morbidity and death rates linked to these disorders, the main concern in the therapy of patients with AP is the development of mild pancreatitis to its more severe stages. It is quite concerning, as people with complex AP are said to have a mortality between 30% and 40%. Aims and Objectives: This study aims to compare levels of C-reactive protein (CRP) and high-sensitivity CRP (hs-CRP) in stratifying risk and predicting outcome in AP. Materials and Methods: A total of 84 patients (sample size) with AP were selected using convenience sampling. Serum amylase, lipase, hs-CRP, CRP, ultrasound abdomen, contrast enhanced computed tomography of abdomen, along with routine blood investigations, were done for the patients. Results: The study included 84 patients, with a mean CRP of 38.27±39.64 mg/dL, and a mean hs-CRP of 105.99±104.99 mg/dL. Receiveroperating characteristic (ROC) curve for CRP analysis showed 66.7% sensitivity and 61.7 % specificity (P=0.079). ROC curve for hs-CRP analysis showed 66.7% sensitivity and 80.2 % specificity (P=0.049) and greater positive predictive value and negative predictive value with hs-CRP than CRP. Conclusion: Hs-CRP is better in stratifying risk and predicting outcome in AP with a better sensitivity, specificity, positive and negative predictive value.
