Abstract: BACKGROUND: The pursuit of techniques that mitigate the vascular risks and patient discomfort associated with the dual arterial access required for conventional retrograde chronic total occlusion (CTO) percutaneous coronary intervention (PCI) is of significant clinical interest. CASE SUMMARY: A 56-year-old man with a CTO of the distal right coronary artery underwent revascularization. After failed antegrade wire escalation technique, a novel single-guiding catheter retrograde technique-the ZHANG Bin technique-was successfully employed. DISCUSSION: The ZHANG Bin technique demonstrates that retrograde CTO-PCI with a single guiding catheter via a single arterial access (radial or femoral) is feasible, enhancing patient safety by reducing vascular complications while maintaining procedural efficacy across specific scenarios. TAKE-HOME MESSAGES: The ZHANG Bin technique represents a potentially significant advancement in procedural efficiency and patient safety. While not universally applicable to all retrograde CTO-PCI cases, it offers a powerful and elegant alternative in specific scenarios.
