Challenges in Assessing Cognitive Frailty and the Implications for Dementia Prevention in a Super-Aging Society

Year: 2026
Type: book-chapter
Abstract: As global populations age, dementia has emerged as a pressing public health concern, making its prevention and early detection increasingly important. Mild cognitive impairment (MCI), a transitional stage between normal aging and dementia, is characterized by cognitive decline with preserved daily functioning and is recognized as a high-risk factor for dementia onset. Of particular interest is “cognitive frailty,” a condition marked by the coexistence of MCI and physical frailty, which is associated with an elevated risk of both dementia and mortality. Despite its significance, standardized evaluation and diagnostic criteria for cognitive frailty remain undeveloped, highlighting the need for effective early detection methods. Recent studies suggest a strong relationship between cognitive and physical function, raising the possibility that physical performance assessments could serve as the early indicators of cognitive frailty. This section explores the current approaches for assessing cognitive frailty, examines the potential of incorporating physical function into such evaluations, and discusses the key challenges involved in developing a new, reliable framework for cognitive frailty assessment.
