Abstract: Sigmoid volvulus is a potentially life-threatening surgical emergency and remains an important cause of large bowel obstruction in older adults. We describe the case of a 60-year-old man who presented with progressive abdominal distension, pain, and vomiting. Radiological evaluation revealed marked torsion and dilatation of the sigmoid colon with features suggestive of ischemia. Emergency surgical exploration confirmed gangrenous sigmoid volvulus, and definitive surgical management was performed. The patient had an uncomplicated postoperative recovery. This report highlights the need for early radiological diagnosis and prompt surgical intervention to prevent catastrophic outcomes in elderly patients.
