Abstract: Introduction: Urolithiasis is a common and recurrent condition marked by the formation of calculi in the urinary tract. It causes painful symptoms and may lead to complications such as urinary obstruction and renal impairment. Although conventional medicine improves outcomes, high recurrence rates demand alternative long-term strategies. Unani medicine offers remedies such as Hajrul Yahood (Lapis Judaicus), known for its antilithiatic, diuretic, and renoprotective properties. Objective: To evaluate the therapeutic efficacy and safety of Hajrul Yahood in the management of urolithiasis. Materials and Methods: This interventional clinical study included 30 patients aged 18–60 years, of either sex, diagnosed with Hasat-e-Bauliyah (renal or ureteric stones ≤10 mm). Participants were administered Hajrul Yahood capsules at a dose of 2 g daily, divided into two doses for 42 days. Follow-ups were conducted on the 14 th , 28 th , and 42 nd days. Subjective parameters (abdominal pain, burning micturition, hematuria, nausea, and vomiting) were assessed at each visit using a grading scale. Objective parameters – evidence of renal stones via ultrasonography (USG), renal function tests, and liver function tests, were evaluated at baseline and the end of the trial. Data were analyzed using appropriate statistical methods. Results: Significant improvements were noted in abdominal pain (90%, P < 0.001), burning micturition (26.67%, P < 0.001), and nausea and vomiting (13.3%, P = 0.024). Hematuria showed a nonsignificant improvement (6.67%, P = 0.112). USG showed a 30% improvement in stone expulsion ( P = 0.01). No adverse events were reported. Conclusion: Hajrul Yahood appears to be a safe and effective alternative treatment for urolithiasis. However, larger randomized controlled trials are required to validate these findings.
