Abstract: Aim: To assess and compare the salivary expression levels of matrix metalloproteinase-1 (MMP-1) in completely edentulous diabetic and nondiabetic individuals, and to evaluate its correlation with glycemic control markers glycated hemoglobin (HbA1c) and random blood sugar (RBS). Materials and methods:This cross-sectional study included 60 completely edentulous individuals aged 40-80 years, divided into two groups: diabetics (n = 30) and nondiabetics (n = 30), classified based on HbA1c and RBS levels.Unstimulated saliva samples were collected under standardized conditions, and salivary MMP-1 concentrations were quantified using enzyme-linked immunosorbent assay (ELISA).Glycemic control was assessed through HbA1c and RBS estimation for all participants.Statistical analysis was performed using the Mann-Whitney U test to compare intergroup differences, and Spearman's rank correlation was applied to determine the relationship between MMP-1 levels and glycemic markers (HbA1c and RBS).Logistic regression analysis was used to identify independent predictors of diabetes and evaluate the diagnostic performance of MMP-1 in differentiating diabetic and nondiabetic individuals. Results: Mean MMP-1 levels were significantly higher in diabetics (3.51 ± 0.26 ng/mL) than in nondiabetics (1.75 ± 0.19 ng/mL; p < 0.05).A strong positive correlation was observed between MMP-1 and both HbA1c and RBS (r ≈ 0.6-0.7;p < 0.05).Logistic regression analysis identified MMP-1, HbA1c, and RBS as significant predictors of diabetic status, yielding a diagnostic accuracy of 87%.Conclusion: Elevated salivary MMP-1 levels are significantly associated with diabetes in edentulous patients and may serve as a potential biomarker for diabetes-related tissue degradation and systemic metabolic status.Clinical significance: The pronounced increase in MMP-1 levels among diabetic edentulous individuals indicates its potential role in diabetesassociated oral tissue deterioration, delayed healing, and alveolar bone loss.Routine monitoring of MMP-1 alongside conventional glycemic markers could enhance early detection of oral health risks and guide preventive care strategies for diabetic patients.
