Abstract: De Quervain’s tenosynovitis (DQT) and carpal tunnel syndrome (CTS) are the common causes of wrist pain, but rarely coexist in patients with inflammatory arthritis (IA). Here, we report the case of a 46-year-old female with IA who presented with 3 months of radial wrist pain and paresthesia in the distribution of the median nerve (MN). Ultrasound (USG) revealed first dorsal compartment tenosynovitis and an MN cross-sectional area of 10 mm 2 at the carpal tunnel inlet. She underwent USG-guided triamcinolone injection for DQT and 5% dextrose hydrodissection for CTS. At 1-month follow-up, DQT-related pain had resolved completely, while intermittent tingling persisted with reduced intensity. USG-guided targeted interventions are safe, precise, and effective for managing peripheral manifestations such as tendon sheath inflammation and nerve entrapments in IA.
