Abstract: Aim. Demonstration of clinical observation of a patient with a long-term history of rheumatoid arthritis with extra-articular manifestations (rheumatoid nodules in the lungs) and comorbid pathology. Key points. Rheumatoid nodules in the lungs are one of the rarest extraarticular manifestations of rheumatoid arthritis. Most often they are asymptomatic, and in most cases their specific treatment is not required. However, when they are detected, it is necessary to conduct a differential diagnosis to exclude a number of diseases (tuberculosis, neoplasia, etc.) and choose the right tactics for managing the patient. Conclusion. Differential diagnosis of lung lesions in patients with rheumatoid arthritis, including the differentiation of infectious (tuberculosis, etc.) and non-infectious (oncological diseases, etc.) processes, remains a difficult clinical task. It requires a comprehensive interdisciplinary approach: assessment of the risk of malignancy of the process, dynamic monitoring, consultations of various specialists. At the same time, a lung biopsy is not a routine diagnostic method. Keywords: rheumatoid arthritis, rheumatoid nodules, differential diagnosis, extraarticular manifestations.
