Abstract: BACKGROUND: Left ventricular pseudoaneurysm (LVPA) is a rare but life-threatening complication that commonly occurs secondary to myocardial infarction, surgery, trauma, and infection. CASE SUMMARY: We report an 80-year-old woman who presented with ischemic chest pain after undergoing percutaneous transluminal coronary angioplasty. Transthoracic echocardiography demonstrated a giant LVPA in the left ventricular inferolateral wall. Although surgical intervention was indicated, we proceeded with conservative management based on patient preference after thorough counseling. The patient was successful discharged and has been doing well on medication therapy. DISCUSSION: This case highlights the importance of timely diagnosis of LVPA despite its low incidence rate to improve patient prognosis. TAKE-HOME MESSAGES: LVPAs are a rare but serious complication after percutaneous transluminal coronary angioplasty. Transthoracic echocardiography helps in diagnostic confirmation, initiation of prompt treatment, and post-therapeutic surveillance.
