Abstract: Native coarctation of the aorta (CoA) accounts for approximately 6%-8% of congenital heart defects and is associated with significant morbidity and mortality if left untreated [1].A functional interruption of the descending thoracic aorta at the isthmus caused by severe coarctation with an atretic or near-atretic lumen is, however, exceptionally rare [2].Despite advances in therapy, the optimal management strategy for native CoA in pediatric patients, surgical or endovascular, remains debated [3,4].We report the case of a 12-year-old boy referred for cardiology evaluation after a routine school health screening.Physical examination showed a systolic murmur in the interscapular region. His history included lower limb pain, reduced exercise tolerance, recurrent headaches, and frequent epistaxis.Blood pressure was markedly elevated in both arms, with a systolic gradient >20 mm Hg between the upper and lower extremities.Upper-limb pulses were bounding, while femoral pulses were weak.Transthoracic echocardiography demonstrated a peak systolic gradient of 42 mm Hg across the aortic isthmus, left ventricular hypertrophy, and
