Abstract: Objective: The action and effect of redox imbalance caused by free radicals and its cause-and-effect relationship with metabolic syndrome were identified in vivo and in real time using capillaroscopy integrated into the ATDM System’s Metabolic Assessment. Methodology: A multicenter, quantitative-documentary observational study was conducted on 6,031 individuals of both sexes, with no age or health restrictions, in Mexico, the United States, the Dominican Republic, Guatemala, Puerto Rico, and Colombia. Bioimpedance and capillaroscopy were performed, an imaging atlas was generated, and five groups of cellular stress were identified. Results: Bioimpedance showed metabolic age deviations consistent with premature aging (18% mild: +3–10 years; 35% moderate: +11–20; 12% high: >20; 1% severe: >30). Capillaroscopy revealed real-time tissue patterns of reactive oxygen and nitrogen species, lipid peroxidation lesions, signs of endothelial dysfunction, and radical-mediated modification of Maillard products and LDL lipoproteins (oxidation, nitrosylation, and hydroxylation). Conclusions: Capillaroscopy, integrated into the ATDM Metabolic Assessment, enabled the non-invasive identification of biochemical redox imbalance processes before clinical manifestation, facilitated risk stratification, and provided causal evidence of metabolic syndrome. It was recommended to standardize interpretation criteria, validate biomarkers, and evaluate their clinical impact and cost-effectiveness in prospective studies.
