290P Immunotherapy-adjusted locoregional complexity score to anticipate surgical difficulty after neoadjuvant immune checkpoint inhibition
Year: 2026
Type: article
Abstract: Neoadjuvant immune checkpoint inhibitors are increasingly used for locally advanced thoracic cancers, but therapy-related inflammation and fibrosis can obscure surgical planes, especially around major mediastinal and hilar vessels. To better anticipate operative complexity in this evolving setting, we developed an Immunotherapy-Adjusted Locoregional Complexity (IALC) score that incorporates anatomic proximity to critical vascular structures and the extent of immune-mediated tissue remodeling.
