Barriers to Isotretinoin Access and Treatment Continuity in an Underserved Pediatric Population: A Cross‐Sectional Study

Year: 2026

Study design: Observational Study
Abstract: Isotretinoin is a highly effective treatment for severe acne, but its use is often complicated by scheduling, pharmacy, and iPLEDGE-related barriers that may contribute to greater administrative burden for patients facing pre-existing structural barriers to care. We conducted a cross-sectional survey study of 42 pediatric patients at an urban safety-net hospital who had completed at least 1 month of isotretinoin, assessing demographics, treatment experience, and barriers to use. Overall, 64.3% of patients reported appointment, pharmacy, or financial-related difficulties, contributing to treatment gaps in 33.3%; nevertheless, 97.6% felt their treatment was successful. Despite high perceived efficacy, isotretinoin use was impacted by administrative and iPLEDGE-related requirements, potentially magnifying existing access barriers.
